Care Management

Each consumer’s needs are assessed individually and a care plan is developed with the consumer, outlining the steps necessary to address those needs. In addition, services include follow-up on consumer referrals and coordination with various service providers.

Assessment

Assessments and reassessments are completed wherever the older adult is located to determine if the applicant is eligible or appropriate for AAA services, nursing or personal care home placement, or waiver services.

PA Department of Aging Waiver Program

The Waiver Program is for people 60 and older who qualify and desire home and community-based long-term services as an alternative to nursing home admission. The program is a special waiver of certain Medicaid restrictions, allowing payments typically used for nursing home care to be used for home-care services. Donations are neither accepted nor solicited for waiver services.

Protective Services

Protective services are provided to incapacitated older adults unable to perform or obtain services that are necessary to maintain physical or mental health, for whom there is no responsible caretaker and who are in imminent danger to person or property. Services are available 24 hours a day and after normal working hours 911 should be called.

Long-Term Care and Ombudsman Advocacy

The Ombudsman Program acts as an advocate for those receiving care in nursing facilities, personal care homes, and long-term care services in the community.

Information and Referral

This service provides information about a variety of services and support needed by older adults and caregivers that is available throughout the area. Information requests should be made by calling (814) 226-4640.

Transportation

Older adults must contact the AAA to be authorized for transportation services, including trips to senior centers, medical appointments, and social service appointments. Additional information may be obtained from the Clarion County Transportation Program at (814) 226-7012 or toll free at 1-800-672-7116.
