Services Offered:

• Care Management: Ongoing care plan management to ensure the older adult's needs are being met.

• Personal Care Services: Assist with activities of daily living including bathing, dressing, and groom.

• Home Delivered Meals: Meals delivered to your home to assist with maintaining nutrition and meal preparation.

• Adult Daily Living Center: A partnership with VNA Extended Care Services that assists older adults seeking socialization, a safe environment, and respite as they continue to live in their own homes.

Eligibility:

• Must live in PA

• Must be a least 60 years of age

• Must be a US citizen or a legal resident

• Must be determined to have unmet needs that impact daily functioning

*Income & Resource Eligibility is needed for Personal Care Services and Adult Daily Living Center. Sliding Scale Sliding Scale is used.
